I Re: 1986 IARC Monograph on the Evaluation of the Carcinogenic Risk of Chemicals to Humans - Tobacco smoking, Vol. 38 This publication represents the "views and expert opinions of an International Agency for Research on Cancer (IARC) Working Group on the Evaluation of the Carcinogenic Risk of Chemicals to Humans" which met in Lyon, France, 12-20 February, 1985. Background The IARC was established in 1965 by the World Health Organization to develop, conduct and evaluate cancer research. The IARC Monographs programme, initiated in 1969 to evaluate carcinogenic risk to humans, has as its objective to provide evaluations "to assist national and international authorities in formulating decisions concerning preventive measures". It notes, however: "No recommendation is given concerning legislation, since this depends on risk-benefit evaluations, which seem best made by individual governments and/or other international agencies." (p. 15, "Preamble") In a number of instances, individuals or groups advocating smoking controls have made reference to this publication• For example, the 1985 report of the Norwegian National Council on Smoking and Health proposing major changes to the Norwegian Tobacco Act, the May 1986 US Office of Technology Assessment staff paper on "passive" smoking in the workplace, and a report on women and "passive" smoking by the Edinburgh-based Association for Non-Smokers' Rights, reportedly made reference to this document. Dr. Grabauskas speaking before the World Health Assembly in May, 1986 also cited this document. Summary The monograph covers 1) worldwide use of smoking tobacco, 2) chemistry and analysis of tobacco smoke, 3) biological data relevant to the evaluation of carcinogenic risk to humans, _., 4} epidemiological studies of cancer in humans. Both active and o "passive" smoking are covered. 2 - In the final chapter, "Conclusions and evaluation" (pp. 309-314), the following evaluations are made: "There is sufficient evidence that inhalation of tobacco smoke as well as topzcal application of tobacco smoke condensate cause cancer in experimental animals. "There is sufficient evidence that carcinogenic to humans. tobacco smoke is "The occurrence of malignant tumours of the respiratory tract and of the upper digestive tract is causally related to the smoking of different forms of tobacco (cigarettes, cigars, pipes, bidis). The occurrence of malignant tumours of the bladder, renal pelvis and pancreas is causally related to cigarette smoking." (p. 314} Specifically on "passive" smoking it states: "The observations on nonsmokers that have been made so far are compatible with either an increased risk from 'passive' smoking or an absence of risk. Knowledge of the nature of sidestream and mainstream smoke, of the materials absorbed during 'passive' smoking, and of the quantitative relationships between dose and effect that are commonly observed from exposure to carcinogens, however, leads to the conclusion that passive smoking gives rise to some risk of cancer."
