Port Washington, NY 11050-0401 Appendix B Dear Member: With this letter I am sending you 5 packs of each of two different test cigarettes which I hope you will enjoy testing for me. I would appreciate your doing this test in the following way: 1. All of these cigarettes should be smoked only by the person whose name appears on the label above. This is because I would like a very select group of people to try these cigarettes. 2. Please start by smoking 3 or 4 packs of the cigarettes coded K2 (and no others). As soon as you have firiished smoking them, please fill out the answers to Part I of the report form on the back of this page. 3. Then smoke 3 or 4 packs of G8 (and no others) and answer Part II of the report form. 4. Finally, smoke some of the remaining cigarettes of both codes K2 and G8 and answer Part III. 5. About ten days from the time you received these cigarettes, we will telephone you to get your reactions to them. Please keep the completed report form handy. This will make it easier for you to tell us about your reactions to the test cigarettes when we telephone you. That's all there is to it!
