To date, we have not received your completed questionnaire. The National Interagency Council is conducting this survey as a first step towards planning effective programs for businesses, as well as gaining insight into the current 'state of affairs' of smoking programs in the workplace. A complete report will be prepared from the responses and will be provided to all corporations which assist us in this effort. All responses to the survey will be kept in strictest confidence; no corporation will be identified in the report unless prior approval is obtained. No doubt you are besieged with requests for information on a daily basis. However, our questionnaire is designed to take very little of your time, and you will certainly be doing us a great service by completing it. Whether or not such programs currently exist in your organization...regardless of your interest in implementing such programs...we need your help.
