PURPOSE: THE RATIO OF CARBON MONOXIDE (C0) TO OXYGEN BOUND BY ADULT
HEMOGLOBIN IS 217 TIMES GREATER THAN THE RATIO OF THESE GAS TENSIONS.
IN AN EQUILIBRIUM MEDIUM, ALTHOUGH THE STRUCTURE OF ADULT AND FETAL
HEMOGLOBIN ARE WELL DEFINED~ THE RELATIVE CO AFFINITY OF FETAL HEMO-
GLOBIN HAS NOT BEEN INVESTIGATED, THIS STUDY DETERMINED FETAL HEMO-
GLOBIN CO AND OXYGEN BINDING VALUES,
~ETHOD~: SAMPLES OF MATERNAL, FETAL, AND PLACENTAL BLOOD WERE OB-
TAINED FROM 15 PATIENTS UNDERGOING CAESAREAN SECTION, AN EQUILIBRA-
TION METHOD WAS USED TO MEASURE CO AND OXYGEN UPTAK~ AND THE RELATI[VE
AFFINITY CONSTANT OF HEMOGLOBIN WAS CALCULATED,
~: FETAL AND PLACENTAL BLOOD HAD A 20% LOWER RELATIVE AFFI-'
NITY FOR CO THAN ADULT (MATERNAL) BLOOD: I,E,J FETAL HEMOGLOBIN HAD
LESS PREFERENTIAL BINDING OF CO OVER OXYGEN THAN ADULT HEMOGLOBIN,
BY CORRECTING FOR THE FRACTION OF ADULT HEMOGLOBIN IN PLACENTAL BLOOD
IT WAS CALCULATED THAT FETAL HEMOGLOBIN HAS A RELATIVE AFFINITY CON-
STANT OF 172 ± 12,
DISCUSSION: BECAUSE OF THE DIFFERENCE BETWEEN ADULT AND FETAL HEMO-
GLOBIN, "THE DIFFUSION GRADIENT FOR C0 ACROSS THE PLACENTA CAN NOT BE
EQUATED WITH THE DIFFERENCE IN CARBOXYHEMOGLOBIN PER CENT SATURATION
(COHB), THIS RELATIONSHIP IS OF INTEREST WHEN COMPARISONS ARE MADE
BETWEEN MATERNAL AND FETAL COHB LEVELS TO DETERMINE THE DIRECTION OF
CO TRANSFER. SUCH COMPARISONS ARE PERTINENT WHEN CO IS USED TO EVAL-
